Student-athletes, in grades 7 through 12 and 6th-grade students planning to participate in softball/baseball, must submit the following before being allowed to participate in their first athletic practice:

1. IOWA ATHLETIC PRE-PARTICIPATION PHYSICAL EXAMINATION FORM -  FORM

2. HEADS UP CONCUSSION FORM - FORM

3. PROOF OF INSURANCE FORM - FORM

***Forms MUST BE submitted yearly to the office in hard copy or uploaded through the online registration process. 

Adult Activity Tickets

Adult Activity Tickets are available for purchase. Contact the district office for the price. These tickets are good for all regular-season home athletic events.
